Transitional Housing

Our Transitional Housing program provides safe, stable, and supportive housing for individuals experiencing homelessness, housing instability, or recovery-related challenges. The program is designed to help clients transition from crisis situations into greater independence while receiving structured support in a clean and recovery-focused environment.

Residents receive housing stability support, wellness check-ins, life-skills assistance, and connections to healthcare, behavioral health, employment, and community resources. Our goal is to create a secure foundation that promotes long-term housing stability, improved health outcomes, and successful reintegration into the community.

Housing Transition Navigation Services

Our Housing Transition Navigation Services help individuals identify, secure, and transition into permanent or longer-term housing. We work closely with clients to assess housing needs, develop individualized housing plans, overcome barriers to housing placement, and coordinate with landlords, housing agencies, managed care plans, and community partners.

Services may include housing searches, application assistance, document preparation, benefits coordination, move-in planning, and referrals to supportive services. The program is designed to help clients successfully navigate the housing process while reducing the risk of ongoing homelessness or housing instability.

Short-Term Post-Hospitalization Housing

Our Short-Term Post-Hospitalization Housing program provides temporary housing and supportive services for individuals who are exiting hospitals, behavioral health facilities, residential treatment programs, nursing facilities, or correctional settings and need a safe environment to continue their recovery. This service is intended for individuals experiencing homelessness or housing insecurity who require additional support following discharge from an institutional setting.

Clients receive a structured and supportive living environment while accessing care coordination, transportation assistance, medication support, wellness monitoring, and connections to medical, behavioral health, and housing resources. The program is designed to reduce avoidable hospital readmissions, support continued recovery, and assist clients in transitioning toward stable long-term housing.

Recuperative Care (Medical Respite)

Our Recuperative Care (Medical Respite) program provides short-term residential care for individuals who are medically stable enough to leave the hospital but still require a safe and supportive environment to recover from illness, injury, surgery, or behavioral health conditions. This service is specifically designed for individuals experiencing homelessness or unstable living conditions whose health could worsen without proper recovery support.

Clients receive a clean, supervised, and recovery-oriented setting with assistance coordinating follow-up medical appointments, transportation, meals, medication support, wellness monitoring, and connections to community resources. Our Recuperative Care program supports healing outside of the hospital setting while improving health outcomes and reducing unnecessary emergency room visits and hospitalizations.
