Recuperative Care That Supports Health, Recovery, and a Path to Independence

Bridgewell Care provides a safe, stable, substance-free environment for individuals recovering from hospitalization, illness, or injury who need additional support to establish safe and permanent housing. Clients receive three nutritious meals daily, medication support, transportation coordination, and assistance following their healthcare and discharge plans.

Through targeted case management and coordination with Enhanced Care Management (ECM) providers, we collaborate with health plans, medical providers, case managers, and community partners to connect clients with primary and specialty care, behavioral health and substance use services, benefits, transportation, and other essential resources.

Our goal is to bridge the gap between hospital and home by addressing barriers to recovery, supporting improved health outcomes, and helping each client move toward greater stability and independence.

Enhanced Care Management

Bridgewell Care provides whole-person Enhanced Care Management (ECM) for individuals with complex health and social needs, helping members coordinate their care, navigate health and social service systems, and access the resources needed to achieve greater health, stability, and independence.

Each member works with a dedicated care manager establishing a therapeutic alliance to develop an individualized, person-centered care plan based on their needs and goals. Our team coordinates across medical care, behavioral health and substance use services, housing and community resources, benefits, and other supportive services—helping members navigate systems that can often be difficult to manage on their own.

Through consistent outreach, care coordination, and ongoing support, Bridgewell Care helps members stay connected to their providers, follow through with care plans, navigate transitions between care settings, and address barriers that may impact their health and well-being.

Housing Transition Navigation Services

Our Housing Transition Navigation Services help individuals identify, secure, and transition into permanent or longer-term housing. We work with members to assess housing needs and preferences, identify barriers, develop an individualized Housing Support Plan, search for appropriate housing, complete applications and documentation, coordinate with landlords and housing agencies, and plan for a successful move into permanent housing.

Services may include housing searches, application assistance, document preparation, benefits coordination, move-in planning, and referrals to supportive services. The program is designed to help clients successfully navigate the housing process while reducing the risk of ongoing homelessness or housing instability.

Housing Tenancy & Sustaining Services

Bridgewell Care helps individuals maintain stable housing and avoid a return to homelessness. We provide personalized support to address challenges that can put housing at risk, including lease compliance, landlord or neighbor concerns, household management, housing crisis prevention and financial or social barriers.

Our team works with members to develop a housing support plan, resolve tenancy issues, connect with benefits and community resources, and communicate with landlords or property managers when needed. Through ongoing support and early intervention of any potential recovery challenges, we help members build the skills and resources needed to remain safely and successfully housed.
